Painkillers And The Kidneys: Analgesic Nephropathy
An analgesic is any medicine intended to relieve pain. Over-the-counter analgesics include aspirin, acetaminophen, ibuprofen, naproxen sodium, and others. These drugs present no danger for most people when taken in the recommended dosage. But some conditions make taking even these common painkillers dangerous for the kidneys. Also, taking one or a combination of these drugs regularly over a long period of time may increase the risk for kidney problems. Most drugs that can cause kidney damage are excreted only through the kidneys.
Analgesic use has been associated with two different forms of kidney damage. Some patient case reports have attributed incidents of sudden-onset acute kidney failure to the use of over-the-counter painkillers, including aspirin, ibuprofen, and naproxen. The patients in these reports had risk factors such as systemic lupus erythematosus, advanced age, chronic kidney disease, or recent heavy alcohol consumption. These cases involved a single dose in some instances and generally short-term analgesic use of not more than 10 days. Acute kidney failure requires emergency dialysis to clean the blood. Kidney damage is frequently reversible, with normal kidney function returning after the emergency is over and the analgesic use is stopped.

A New Study About Coffee And Migraines: How Much Is Too Much
In a new study published in the American Journal of Medicine, researchers asked 98 people with migraines to keep a diet diary that included how often they consumed caffeinated beverages . This information was compared with how often they had migraines. Heres what they found:
- The odds of having a migraine increased for those drinking three or more caffeinated beverages per day, but not for those consuming one to two servings per day the effect lasted through the day after caffeine consumption.
- It seemed to take less caffeine to trigger a headache in those who didnt usually have much of it. Just one or two servings increased the risk of migraine in those who usually had less than one serving per day.
- The link between caffeine consumption and migraine held up even after accounting for other relevant factors such as alcohol consumption, sleep, and physical activity.
Interestingly, the link was observed regardless of whether the study subject believed that caffeine triggered their headaches.
Is The Combination Of Ibuprofen And Caffeine Effective For The Treatment Of A Tension

Diamond S, Balm TK, Freitag FG. Ibuprofen plus caffeine in the treatment of tension-type headache. Clin Pharmacol Ther 2000 68:312-19.
BACKGROUND: Studies have shown the effectiveness of the addition of caffeine to ibuprofen for acute pain. However, this combination for use in tension-type headaches has been less well studied because of insensitive methods for evaluating this type of headache pain. The purpose of this study was to evaluate the efficacy of ibuprofen plus caffeine for the treatment of tension-type headaches and to establish a sensitive method to compare the combination versus either agent alone.
POPULATION STUDIED: This multicenter study reported on 301 evaluable patients with a history of tension-type headaches as defined by the International Headache Society. The mean age of the patients was 37 years. Patients were included who had 3 to 15 tension-type headaches every month for the past year and were responsive to over-the-counter analgesics at least 75% of the time. Patients who also suffered from migraines were able to participate if the type of headache pain could be differentiated by the quality of the pain or associated symptoms. Patients were excluded if they were allergic to the study drugs or had a comorbid illness that could potentially affect the pharmacokinetics of the study medication.

Caffeines Effects On Pain And Non
There is evidence that caffeine may reduce pain sensation through its effects on adenosine receptors . The antinociceptive effects of caffeine may be explained by an inhibition of cyclooxygenase activity as well as adenosine receptor antagonism. Caffeine acts not only by central blocking of adenosine receptors, which affects pain signaling, but also by blocking peripheral adenosine receptors on sensory afferents . It was demonstrated that a 200 mg caffeine dose can inhibit the analgesic effects of transcutaneous electrical nerve stimulation . Caffeine combined with a standard dose of analgesics led to an increased proportion of individuals with a satisfactory level of pain relief . Laska et al. found that, in combination with paracetamol or aspirin, caffeine reduced the amount of analgesic needed to reach the same effect by approximately 40% . Other clinical effects in these patients may be linked with the promotion of the absorption of analgesics by rapid lowering of gastric pH. Nevertheless, meta-analyses of caffeine combined with ibuprofen, paracetamol, or acetylic acid found only weak adjuvant effects in patients with postoperative pain .

Pain Reliever Linked To Kidney Injury In Endurance Runners
The common practice of taking ibuprofen for pain relief while competing in ultramarathons causes an increased risk of acute kidney injury, a Stanford study says.
Grant Lipman and his colleagues found that runners who took ibuprofen during ultramarathons doubled their risk for kidney injury.Paul Sakuma
People who take the painkiller ibuprofen while running very long distances double their risk of acute kidney injury, according to a study by researchers at the Stanford University School of Medicine and several other institutions.
As many as 75 percent of ultramarathoners use the nonsteroidal anti-inflammatory drug, or NSAID, in this fashion, according to Grant Lipman, MD, clinical associate professor of emergency medicine at Stanford and director of Stanford Wilderness Medicine. And while most cases of acute kidney injury appear to resolve spontaneously, the condition has the potential to progress to renal failure, he said.
Lipman is lead author of the study, which was published online July 5 in Emergency Medical Journal. Brian Krabak, MD, a sports and rehabilitation medicine specialist at the University of Washington-Seattle, is the senior author.

Coffee And Painkillers: Why Not To Worry
The plain truth behind mixing the two, and how recent news reports got it wrong
In the last week of September 2007, a new study was published by the University of Washington School of Pharmacy on toxic interactions between caffeine and acetaminophen, the painkiller chemical thats the active ingredient of Tylenol, Excedrin, and other OTC pain-relieving and cold remedy products.
A handful of media outlets wasted little time in sounding an alarm. Consuming coffee and Tylenol at the same time may cause liver damage, warned an online Fox News article. U.S. News and World Report had UWSP dean and study coauthor Sid Nelson, PhD, suggesting use of aspirin or ibuprofen if you find yourself dealing with a Red Bull-vodka headache , and the pharmacy school reproduced this article on its Web site. A Canadian university student newspaper even went so far as to use the headline: The hangover killer that is killing you.
Dont let these sensationalized headlines scare you. A closer look at the actual study showed that creation of a toxic liver byproduct occurred when researchers used a caffeine amount equivalent to 20 cups of coffee. If you belong to this over-caffeinated group, you should talk with your doctor about alternative OTC pain relievers .
For the rest of us, its safe to drink moderate amounts of caffeine while taking acetaminophen as directed.
Can I Take An Aspirin After Drinking
Mixing aspirin and alcohol can result in certain types of gastrointestinal distress. Aspirin can cause nausea and vomiting when mixed with alcohol. Does Caffeine Really Boost Painkiller Effectiveness
A new review finds this popular pain medication additive brings a small but significant amount of relief.
FRIDAY, Mar. 16, 2012 Whether theyre getting it in coffee, tea, colas, or chocolate, theres no doubt that people love their caffeine. This popular stimulant affects the central nervous system, which is why it helps keep you alert and energetic. Caffeine is also an ingredient in many over-the-counter and prescription pain medications but does it really work to ease aches?
To answer this question, The Cochrane Library reviewed 19 studies involving more than 7,200 patients taking medications to relieve a variety of pain conditions, including postpartum pain and headache. All the studies compared the effects of taking a single dose of a pain reliever like acetaminophen or ibuprofen versus the same dose with added caffeine. The researchers found that 5 to 10 percent more of the people who took pills with caffeine experienced a good level of pain relief.
So now we know that the addition of caffeine can fight pain better than a painkiller alone but we still dont know why. Sheena Derry, one of the authors of the current study, told Health Behavior News Service that the stimulant could help painkillers enter the bloodstream more quickly or slow their clearance from the blood, or it could exert a mood-boosting effect that eases pain. Although the size of the effect is small, it is probably clinically useful, she added.
