A New Study About Coffee And Migraines: How Much Is Too Much

In a new study published in the American Journal of Medicine, researchers asked 98 people with migraines to keep a diet diary that included how often they consumed caffeinated beverages . This information was compared with how often they had migraines. Heres what they found:

• The odds of having a migraine increased for those drinking three or more caffeinated beverages per day, but not for those consuming one to two servings per day the effect lasted through the day after caffeine consumption.
• It seemed to take less caffeine to trigger a headache in those who didnt usually have much of it. Just one or two servings increased the risk of migraine in those who usually had less than one serving per day.
• The link between caffeine consumption and migraine held up even after accounting for other relevant factors such as alcohol consumption, sleep, and physical activity.

Interestingly, the link was observed regardless of whether the study subject believed that caffeine triggered their headaches.

BACKGROUND: Studies have shown the effectiveness of the addition of caffeine to ibuprofen for acute pain. However, this combination for use in tension-type headaches has been less well studied because of insensitive methods for evaluating this type of headache pain. The purpose of this study was to evaluate the efficacy of ibuprofen plus caffeine for the treatment of tension-type headaches and to establish a sensitive method to compare the combination versus either agent alone.

POPULATION STUDIED: This multicenter study reported on 301 evaluable patients with a history of tension-type headaches as defined by the International Headache Society. The mean age of the patients was 37 years. Patients were included who had 3 to 15 tension-type headaches every month for the past year and were responsive to over-the-counter analgesics at least 75% of the time. Patients who also suffered from migraines were able to participate if the type of headache pain could be differentiated by the quality of the pain or associated symptoms. Patients were excluded if they were allergic to the study drugs or had a comorbid illness that could potentially affect the pharmacokinetics of the study medication.

People who take the painkiller ibuprofen while running very long distances double their risk of acute kidney injury, according to a study by researchers at the Stanford University School of Medicine and several other institutions.

As many as 75 percent of ultramarathoners use the nonsteroidal anti-inflammatory drug, or NSAID, in this fashion, according to Grant Lipman, MD, clinical associate professor of emergency medicine at Stanford and director of Stanford Wilderness Medicine. And while most cases of acute kidney injury appear to resolve spontaneously, the condition has the potential to progress to renal failure, he said.

Lipman is lead author of the study, which was published online July 5 in Emergency Medical Journal. Brian Krabak, MD, a sports and rehabilitation medicine specialist at the University of Washington-Seattle, is the senior author.

To answer this question, The Cochrane Library reviewed 19 studies involving more than 7,200 patients taking medications to relieve a variety of pain conditions, including postpartum pain and headache. All the studies compared the effects of taking a single dose of a pain reliever like acetaminophen or ibuprofen versus the same dose with added caffeine. The researchers found that 5 to 10 percent more of the people who took pills with caffeine experienced a good level of pain relief.

So now we know that the addition of caffeine can fight pain better than a painkiller alone but we still dont know why. Sheena Derry, one of the authors of the current study, told Health Behavior News Service that the stimulant could help painkillers enter the bloodstream more quickly or slow their clearance from the blood, or it could exert a mood-boosting effect that eases pain. Although the size of the effect is small, it is probably clinically useful, she added.
